Bottoming Out After Breast Implants

Bottoming Out After Breast Implants – What are the Causes?

What is bottoming out after breast implants?

Bottoming out may come into act when the breast implants use to slip below the natural and surgical process. Keep in mind that the fold line will remain just under the breasts where it uses to meet the chest portion. This sort of symptoms often uses to include a lower crease at the breast. In such cases the areola and the nipple portion will tilt upwards and at the same time the infra-mammary scars at the breast region will move up towards the lower breast mound. Patient may come across a risk for bottoming out if there is an over dissection at the implant pocket. This use to occur during the surgery, thus expert surgeon needs to handle this sort of complex surgical process so that patients will not come across further problems.

This is frequently misunderstood concept and even a few plastic surgery web sites will get that wrong! Traditionally, the bottoming-out was term applied while lower half of breast skin to be stretched & lengthened out after the traditional “invertedT” style reduction surgery. The lower pole stretch can give characteristic look to breast, with following features:

1) illusion of overly high placement of nipple,

2) vertically oblong shaped of breast,

3) loss of the upper pole fullness with the excess fullness in lower half of breast (“teardrop” than “round”),

4) upward oriented nipple.

Usual treatment was surgically removing stretched out skin, making use of horizontal ellipse style, placed in infra mammary crease region. It fixes problem nicely, particularly in women, which have the scar in crease. Currently, same term is applied to some complications after the breast implant surgery, as wlel as this is where terminology at times goes astray. Also, 2 situations will look same to untrained eye….

A) Expansion of pocket. In the situation, inframammary crease also descends to abnormally low position & implant falls downward in lower position with the gravity. Search for distance between original incision (in case, it was in crease) and crease itself (that is moving downward) being greater than it was.

B) True bottoming out. So, here t crease level are fixed and scar position doesn´t migrate. Skin of lower part of breast is ballooning and as before, skin tightening operation – modified mastopexy – is an usual treatment.

Why it is important to make distinction?

Because treatments are very different and treatment of the pocket expansion generally involves steps in operating room. Ensure pocket is not at all scarred down and encapsulated in upper part of breast.
